Insurance

United Behavioral Health
Our Licensed Professional Counselor/Therapist is an in network provider for United Behavioral Health.  UBH will provide payment for "Traditional" psychotherapy, provided in an office setting using proven methods and modalities.  Does not recognize equine assisted psychotherapy, or equine assisted experiential learning as a proven modality and will not pay for these services.  If your policy requires a copay or deductible, payment will be expected at the time service is rendered.

Medicaid
NC Health Choice
  If your insurance is either Medicaid or NC Health Choice, these insurances will allow up 13 unmanaged visits per year per client.  These insurances will pay for "traditional" counseling sessions but do not recognize or reimburse for Equine Assisted Psychotherapy, or Equine Assisted Experiential Learning Sessions. Applicapable copays are due at the time service is rendered.  It is the client's responsibility to ensure that their policy is in effect at time of service or be prepared to render payment for service.

Blue Cross Blue Shield
Our therapist is an out of network provider for BCBS.  We will be happy to provide you with a CMS 1500 document which you can use to file for reimbursement but payment for service is due as service is rendered.  BCBS does not recognize either Equine Assisted Psychotherapy or Equine Assisted Experiential Learning as a proven modality and will not reimburse for these services.

We will be happy to discuss your insurance and treatment needs at anytime.  Feel free to call to discuss your options and what best meets your needs.  If you have insurance other than those listed, we will be happy to explore your options with your insurance company.

Community Organizations

Jake's Flat Farm contracts with a variety of community organizations and agencies to provide Equine Assisted Psychotherapy and Equine Assisted Experiential Learning to their clients.  The following is a partial list of organizations we contract with or have provided service for.  If you are interested in our services and feel that your organization would be willing to contract for our services please feel free to call us for further information.

Rape Crisis Center- contracts for sessions for after care of rape and sexual abuse survivors

Family Net- contracts for Equine Experiential Learning Sessions for Teen Up participants

Hickory City Schools- Contracts for Equine Experiential Learning Workshops for Rising Star participants

Community Schools- Contracts for Equine Experiential Learning Activities for Conover School.

Scholarships and Sliding Scale

We have a number of scholarships available for our summer camps and for our therapeutic riding program.  These scholarships are awarded by Carolina HoofPrints on a need and merit basis. Scholarship Applications are available on our forms page.  If you have questions or a special circumstances which you feel could be assisted by participation in one of our programs but believe that cost is a barrier to participation, feel free to call for additional information.

We offer an income based sliding scale to reduce the cost of our services for individuals who would benefit but do not have the resources to render payment at time of service.  Call us to discuss your needs and the assistance you feel you would need in order to participate.
