Jake's Flat Farm,LLC accepts a variety of payment forms, including private pay, sliding scale arrangements, scholarships, and insurance. We encourage clients to discuss individual financial arrangements.    

 Our Therapist, Elisabeth Bliss-Jaynes, LPC is a Provider for the following insurance:

 Medicaid

United Behavioral Health

Carolina Access

Individual insurance and payment plans are billed for services according to their guidelines and approved services.  Not all of our avaliable therapy modalities are eligible for insurance reimbursement. Please contact our therapist or office manager to discuss your insurance or payment plan.

Jake's Flat is a contract provider for community agencies such as the Rape Crisis Center of Hickory, NC.

Scholarships, sponsorships and Fee Waivers- For clients and students who wish to participate or access services we are able to offer limited scholarships or sponsorships to fund part or all of their sessions. These funds are made available through individual donations, or organizations such as Carolina Hoofprints.  If an individual is interested in a scholarship or being sponsored in order to participate, please contact us to apply.   Fee waivers are sometimes available on a sliding scale based on family income or special circumstances.

 

 

If you or your organization are interested in sponsoring a person for any of the services provided by Jake's Flat Farm or making a donation of materials or funds we encourage you to contact Liz at  jaynesliz@yahoo.com or give us a call.  Both Carolina Hoofprints and NC Personal Ponies are non-profit organizations and welcome your support as they assist our clients and their families.

 

Volunteer program-  Jake's Flat Farm accepts volunteers from the community for individual work within our program.  Volunteers provide a wide range of support and activity to our clients depending on their expertise and interest.  Volunteer activities may include basic horse care, barn managment, construction projects, trail development, sidewalking or leading horses during sessions, or work as a skilled rider or instructor.  If you are interested in volunteering at Jake's Flat Farm you must be 16years or older, be able to pass a background check and complete our interview and assessment process.  Volunteers are selected on the basis of their skill, experience and best fit for our program by our barn or program directors.  Our volunteer pool currently includes adult community members, and parents, high school and college students.  Volunteering at Jake's Flat Farm, allows individuals unique opportunities to share our experience with our horses and clients while providing valuable service to our community.  We are grateful for their hard work and generous efforts.
